Prior to beginning competition, practice, or team workouts all student-athlete will need to complete all necessary medical paperwork. Click on links below:
All student-athletes are required to complete online medical paperwork. The instructions and link to the proper website are included below. The documentation is required for all student-athletes PRIOR to participation.
2024 - Medical Paperwork Instructions
Student-athletes must be cleared each year through a preparticipation physical exam (PPE) . Preparticipation exams are valid for one calendar year and must be renewed each year as required by the NJCAA. Student-athletes will need to have the exam performed by a Doctor (MD or DO), Physician Assistant, or Nurse Practitioner at their own expense. The PPE must be completed on NJCAA forms and signed by the healthcare practitioner that completed the exam.

In addition to the PPE student-athletes and their parent or guardian will need to complete the Student-Athlete Medical Insurance and Emergency Contact form. Rochester Community and Technical College requires that all student-athletes have Primary Health Insurance while participating in intercollegiate athletics. RCTC does not cover medical expenses for athletes.
